Effectiveness of Focal Pancreatic Parenchymal Atrophy in Diagnosing High-Grade Pancreatic Intraepithelial Neoplasia/Carcinoma in Situ

چکیده
To improve the poor prognosis of pancreatic ductal adenocarcinoma (PDAC), diagnosis early-stage PDAC is essential. In particular, high-grade intraepithelial neoplasia/carcinoma in situ (HG-PanIN/CIS) best option. However, it almost impossible to directly observe HG-PanIN/CIS. Thus, identifying a secondary imaging finding due disorder important. Focal parenchymal atrophy (FPPA) and hypoechoic area have been reported as preferred signs. We studied 50 patients clarify effectiveness FPPA diagnosing Most had opportunity undergo further examination presence cyst. Among patients, 23 (46%) positive results for serial pancreatic-juice aspiration cytologic (SPACE), which has high sensitivity specificity PADC; 20 (87.0%) underwent surgery resect part including FPPA. Distal pancreatectomy pancreatoduodenectomy were performed 19 one patient, respectively. 13 (65%) histopathological revealed HG-PanIN/CIS epithelium resected specimens. could indicate HG-PanIN/CIS, but not satisfactorily. One factors unsatisfactory might be difficulty head area. On other hand, cyst, especially FPPA, lead The size cyst does affect

Microinvasion of High-Grade Pancreatic Intraepithelial Neoplasia
High-grade pancreatic intraepithelial neoplasia (PanIN-3) is recognized as a precursor lesion of invasive ductal carcinoma (IDC). However, histological evidence that PanIN-3 invades beyond the basement membrane of pancreatic ductal epithelium, that is, the moment PanIN-3 becomes IDC, has not been captured yet. This may be because PanINs which are microscopic papillary or flat lesion rarely deve...
متن کاملOverexpression of fibroblast growth factor receptor 4 in high-grade pancreatic intraepithelial neoplasia and pancreatic ductal adenocarcinoma.
The overexpression of fibroblast growth factor receptor (FGFR) 4 has been reported in various human cancers, but it has not been studied in pancreatic ductal adenocarcinoma (PDAC) or its precursor lesion, pancreatic intraepithelial neoplasia (PanIN). Moreover, there is controversy as to whether FGFR4 has a mitogenic role in carcinogenesis or other functions. Therefore, the expression and roles ...
متن کاملUpdate on pancreatic intraepithelial neoplasia.
Pancreatic intraepithelial neoplasia (PanIN) is a histologically well-defined precursor to invasive ductal adenocarcinoma of the pancreas. PanINs are remarkably common lesions, particularly in the elderly population. Molecular studies have helped establish the progression of PanIN to invasive cancer, and recently genetically engineered mouse models have been generated that recapitulate the enti...
متن کاملTranspapillary drainage of pancreatic parenchymal necrosis
In the last two decades the strategy of treatment of necrotizing pancreatitis has changed. Endoscopic therapy of patients with symptomatic walled-off pancreatic necrosis has a high rate of efficiency. Here we present a description of a patient with parenchymal limited necrosis of the pancreas and a disruption of the main pancreatic duct. In the treatment, active transpapillary drainage of the p...
متن کاملPresence of pancreatic intraepithelial neoplasia-3 in a background of chronic pancreatitis in pancreatic cancer patients
The clinical significance of pancreatic intraepithelial neoplasia (PanIN) lesions in non-neoplastic pancreata of pancreatic ductal adenocarcinoma (PDAC) patients remains controversial. As chronic inflammation has been recently demonstrated to promote dissemination of in situ precancerous lesions, we investigated the prognostic significance of PanINs associated with chronic pancreatitis (CP) in ...
